A Lady can be a ruler of a Town. Most of the time the Lord, the Lady's husband rules the Town. An example of a Lady leading a Town was in the Town of Mirefall. Sir Uwen of House Soggs was working as the Knight Commander for King Redford of House Remont in Wandermere at the time, so she was the ruling Lady of the Town. Usually, once the once the ruling Lady or Lord dies, the next heir is the oldest son alive. If there is not a son then a daughter will become the ruler. Even the children are called Lord and Lady even before they come into power. This is for respect of the higher house in authority.
